Description:
Lifecourse Health Development of Individuals with Chronic Health Conditions: Visualizing a Preferred Future

Lifecourse health development (LCHD) is an emerging biopsychosocial model that conceptualizes health development as a nonlinear, adaptive process that occurs through person-environment transactions over time. LCHD aims to reconcile biomedical and psychosocial models with emerging research in epigenetics, psychosocial determinants of health, and timing of life experiences. Knowledge of person-environment transactions that are important for health development and the timing of these transactions are critical to early and continuing interventions for individuals with chronic health conditions. LCHD encourages planning for the future; physical, emotional, and spiritual wellness; primary and secondary prevention; and opportunities and experiences in 'real-life' settings. LCHD promotes coordination and continuity of healthcare as well as social and community supports and services. This presentation will propose that a LCHD approach can be transformative in enabling healthy living and desired social participation of individuals with chronic health conditions throughout their lives. Expanded and innovative roles of physical therapists will be addressed.
