Physical therapy services continue to be on the audit radar for various Medicare administrative contractors and private insurance carriers' medical review entities. This session will review the necessary components of documentation for the progress report, daily notes, and discharge report and help you document the key points to support medical necessity and the skilled nature of physical therapy services. The speaker will address the importance of documentation that demonstrates what the physical therapist or physical therapist assistant did for the patient during a treatment session to support necessity of physical therapy services. Documentation tips will be provided to support the skilled nature of the most common CPT codes billed under a physical therapy plan of care.

Learning Objectives: Upon completion of this course, you will be able to:

1. Identify reasonable and necessary criteria for skilled physical therapy services.

2. Describe and implement the necessary components of a progress report to support the therapy already provided and the need for continued skilled physical therapy services.

3. Apply strategies to more effectively support medical necessity and skilled services in your daily notes.

4. Describe and implement the necessary components of a discharge report to demonstrate the nature of skilled services provided to support why you should be reimbursed for those services.
